A stroke leads to a temporary incompatibility with driving. The driving license is not revoked, but its validity is effectively suspended: the holder cannot get behind the wheel without obtaining a favorable medical opinion, followed by an administrative procedure on the ANTS website. The process varies depending on the severity of the stroke and whether it is accompanied by a prior administrative sanction.
Medical resumption under supervision or reallocation of the license: two distinct processes
The most common confusion after a stroke concerns the nature of the procedure itself. In most cases, it involves a medical fitness assessment unrelated to an infraction. The license still exists, but it is necessary to prove that the health condition allows for driving again.
The situation changes radically if the stroke occurs while the license was already suspended, invalidated, or revoked for another reason (alcohol, drugs, total loss of points). In this case, resuming driving combines two procedures: the medical validation post-stroke and the reallocation procedure related to the sanction. Mandatory psychotechnical tests are then added to the medical examination, and the visit falls under the departmental medical commission, not a simple approved doctor in an office.
To better understand the validity of the driving license after a stroke, one must first identify which administrative process one is in: medical fitness alone, or medical fitness combined with reallocation.
Approved doctor or medical commission: who to consult after a stroke
The choice of practitioner is not free. It depends on the reason for the visit.

When the stroke is the sole reason for the assessment (no sanction, no infraction related to alcohol or drugs), the visit is made to an approved doctor by the prefecture. These practitioners work in private practice. Their list can be consulted on the website of the prefecture of residence or on platforms like Doctolib, filtering by the specialty “approved medicine for driving licenses.”
If the case involves a history related to alcohol or drugs, the visit falls under the departmental medical commission. This commission meets at the prefecture or sub-prefecture, with often limited slots and longer waiting times.
The approved doctor or the commission evaluates several dimensions:
- Motor and sensory abilities, including limb strength and coordination necessary for vehicle maneuvers
- Visual field and visual acuity, often impaired by a stroke affecting the occipital areas
- Cognitive and behavioral functions: reaction time, divided attention, ability to manage multiple pieces of information simultaneously
Depending on the severity of the aftereffects, the doctor may require additional assessment at a specialized center before rendering their opinion.
Incompatibility period according to the severity of the stroke
The decree of August 31, 2010, which sets the conditions for driving fitness, distinguishes two main situations.
After a minor stroke that did not require rehabilitation, resuming driving can be considered after a minimum period of fifteen days. An assessment of visual, sensory, motor, and cognitive disorders must first be conducted, followed by a consultation with an approved doctor.
After a moderate to severe stroke that required rehabilitation, the minimum period is one month. A multidisciplinary assessment of driving abilities is then recommended, if not essential. This assessment can be prescribed by the treating physician, a neurologist, or a rehabilitation doctor.
In both cases, the approved doctor may attach a limited validity period to their favorable opinion, which requires periodic checks (often annually) to verify that the abilities remain compatible with driving.
Administrative procedure on the ANTS website after the medical opinion
The favorable opinion from the approved doctor is not enough to make the license valid. This is a point that many drivers overlook. The medical opinion alone does not authorize resuming driving.
Once the opinion is obtained, a request for renewal or regularization of the license must be made on the ANTS website (National Agency for Secure Titles). The procedure is done online and requires:
- The Cerfa form for the driving license application
- The favorable medical opinion issued by the approved doctor or the commission
- A compliant digital identity photo
- A recent proof of residence
The new license may include restrictive codes indicating mandatory adaptations (driving with an automatic transmission, additional rearview mirror, geographical limitation). These codes appear on the back of the license and can be verified in case of a roadside check.

Car insurance and declaration obligation after a stroke
The insurance code requires the driver to declare any change in their health status that may increase the insured risk. A stroke falls into this category. Failure to declare may lead to a nullification of the contract or a refusal of compensation in case of a claim.
The declaration is made by registered mail or through the insurer’s client space. The insurer can then maintain the contract as is, apply a surcharge, or terminate the contract with notice. In practice, a favorable medical opinion and a regularized license facilitate the maintenance of coverage.
A driver who resumes driving without completing the ANTS procedure drives with a license whose administrative validity is not established. In case of an accident, this situation exposes them to a refusal of coverage by the insurance, in addition to criminal penalties for driving without a valid license.
Resuming driving after a stroke relies on a precise sequence: incompatibility period, medical visit with the right contact, ANTS request, then declaration to the insurer. Skipping a step does not save time but creates a legal void that reveals itself at the worst moment.
